The British broadcaster engaged in a "serious breach" of broadcasting rules by failing to disclose that the voice of a film about Gaza was the child of a Hamas representative, according to regulatory body Ofcom.
An investigation by the broadcasting watchdog into Gaza: How to Survive a Warzone has concluded that the programme was "materially misleading".
The head of the BBC had previously apologised, stating there had been "a major oversight in relation to accuracy".
Ofcom has directed the BBC to air a peak-time statement about its decisions.
"Since this amounts to a serious breach of our regulations, we are instructing the broadcaster to broadcast a announcement of our findings against it on BBC Two at nine o'clock in the evening, with a date to be determined," it said.
'Risk to Erode Credibility'
The authority's statement said: "Our probe found that the programme's lack of disclosure that the narrator's male guardian held a role in the Hamas-run authority was substantially inaccurate.
"It meant that the public did not have vital facts which may have been very significant to their judgment of the presenter and the content he shared.
"Confidence is at the core of the connection between a broadcaster and its viewers, especially for a public service broadcaster such as the public broadcaster.
"This failing had the potential to erode the very considerable degrees of credibility that audiences would have placed in a documentary produced by the BBC about the conflict in Gaza."
The film was withdrawn from the online platform in February after the presenter's familial connections were revealed, and in the month of July an broadcaster's internal assessment determined it breached the organization's editorial guidelines on truthfulness.
A corporation official said: "The Ofcom ruling is in line with the results of the corporation's inquiry, that there was a major oversight in the programme in relation to the BBC's Editorial Guidelines on factual correctness, which aligns with regulation 2.2 of Ofcom's Broadcasting Code.
"We expressed regret for this and we agree with the authority's determination in complete. We will adhere to the penalty as soon as the timing and wording are determined."
